If you find yourself at the last minute, you can bring, email or send this form to Community Village c/o of Janet Tarver 1455 Wilson Court Eugene (near 14th and City View) or planetbubblesuniverse@yahoo.com. The eligibility meeting is open to all village members and will be on the 19th of May at 7pm. Kids and SO's  need to be on your form. The form must arrive in time for the meeting at Janet's house. Mailing it to the Community Village may mean that it will not reach your booth coordinator by the deadline. You are responsible.

PARTICIPATION GUIDELINES Once a person has met eligibility, each person must meet the following guidelines for a camping pass. Attendance of at least two Community Village meetings, one must be before eligibility deadline in May. Daily staffing of booth, workshop presentation, project display, or helping with other areas, hours worked should total 4 hours per day during the Fair. Participate in 2 sweeps, at least one Saturday at the Fair. Attendance and work at two work parties, per pass received. The booth must have consensus on exempting a person from work party duty and be willing to pick up the slack for them. Each booth shall determine through consensus who, among eligible participants receives an available pass. For pass allocation only, "booth" is defined as those who participated in the booth the previous year. Groups and Individuals participate in the Village by conduction of workshops, demonstrations, staffing their booth and sharing information with the public. Groups and Individuals cannot publicly promote a candidate, ballot measure, or specific religion (workshop, handouts, etc.) in their booth area while at the Fair. No Selling of any items in booths (this can only happen in the Fruit booth & Restaurant), donations can be accepted but an amount of the donation cannot be specified. A work party consists of 4 hours of work on your booth or village project. Work parties are for the 4 weekends pre-fair.
Each group is permitted a maximum of: 2 worker passes (wristbands) and 3 day passes, OR 1 worker pass and 6 day passes, OR 9 day passes. Day passes are of one specific day only. Please indicate which day/s you want day passes. Please list all representatives in your group, including yourself, requesting passes. List any Significant Other passes or CV Teen passes you request, include the name of the person. List any Day passes requested, include names. List any vehicle passes you request if camping on site.
NOTE: This is an eligibility form and does not guarantee you will get passes
RETURN THIS FORM TO YOUR BOOTH COORDINATOR BY THE MAY, 2007 COMMUNITY VILLAGE GENERAL MEETING. Mailing it to the Community Village may mean that it will not reach your booth coordinator by the deadline. You are responsible to get your eligibility form to your booth coordinator by the deadline if you want to be considered for a pass.
